[Series: USM Product Insights] VPEARL INTERLOCKING NAIL SYSTEM – A SOLID FOUNDATION FOR BONE AND JOINT RECOVERY

Long bone fractures (femur, tibia, humerus, etc.) are common injuries in orthopedics. The choice of fixation method depends on the fracture location, fracture pattern, bone quality, and patient age. Interlocking nails are placed within the medullary canal, sharing mechanical load with the bone and facilitating early patient mobilization. Consequently, this is a widely favored option and is considered the gold standard in the treatment of femoral and tibial shaft fractures.

The VPEARL Interlocking Nail by USM Healthcare is an integrated implant system comprising nails and locking screws, engineered as a complete platform rather than an isolated product line. The system encompasses anatomically specific nails, locking screws, end caps, and dedicated instrumentation with high-precision targeting jigs.

1. Product Portfolio

The VPEARL system features 11 nail groups categorized by anatomical location:

Each group is available in an extensive range of sizes, enabling surgeons to select the optimal fit for each clinical case.

2. Design and Biomechanical Highlights

2.1. Proximal Femur: One Nail, Two Locking Options

The anti-rotation proximal femoral nail is a cannulated design that allows surgeons to choose between two fixation options on the same platform:

  • Femoral Neck Lag Screw (GAMMA type): A threaded cylindrical screw passing through the femoral neck into the head.
  • Anti-rotation Helical Blade (PFNA type): A flat blade with a broad surface area that provides fracture compression, robust purchase, and superior femoral head anti-rotation control.

The helical blade is especially preferred in osteoporotic bone. Upon insertion, it compacts the surrounding cancellous bone, significantly increasing hold compared to conventional threaded screws—a critical benefit in elderly patients.

Other features of this category:

  • Dual-screw configuration: Uses two combined femoral neck screws to enhance rotational stability.
  • Asian-fit dimensions: Screw diameters are tailored to match Asian bone morphology.
  • Broad configurability: Multiple neck-shaft angles, left/right options, self-tapping locking screws, and end caps in varying lengths.
  • Stress-relief grooves: Longitudinal flutes along the distal tip help dissipate mechanical stress. These flutes are also integrated into the standard femoral and reconstruction femoral nails.

2.2. Dynamic and Static Locking

The distal ends of multiple lines (proximal femoral, standard femoral, and retrograde femoral nails) feature both static and dynamic locking holes.

  • Static Locking: Locks the nail rigidly to the bone, preventing axial migration and rotation. This approach is typically indicated for unstable fractures.
  • Dynamic Locking: Uses an elongated slot allowing axial sliding and interfragmentary compression during weight-bearing, which stimulates bone union.

Surgeons can select either locking modality or combine them, adjusting construct stability according to fracture pattern and healing stage.

3. Specialized Lines

3.1. Multidirectional Tibial Nail

The proximal end features dynamic and multidirectional static locking holes across various planes and angles. This delivers superior flexibility when managing proximal, diaphyseal, and distal tibial fractures, including complex comminuted cases. The system includes cancellous locking screws tailored for proximal tibial bone stock and self-tapping locking screws.

3.2. Retrograde Nails (Femur and Tibia)

Retrograde nails are inserted from the distal end of the bone (transarticular/intercondylar knee approach for the femur) rather than the proximal entry site. This technique is optimal for distal fractures where antegrade nailing cannot achieve secure distal fixation. The retrograde femoral nail is indicated for distal femoral fractures, while the retrograde tibial nail is anatomically contoured to the distal tibia.

3.3. Pediatric Elastic Nails

Featuring a flexible, easily contourable profile, these nails are designed for small-diameter long bones in pediatric and adolescent patients. They come in a comprehensive range of diameters to match canal dimensions, accompanied by multiple end cap sizes.

Because preserving open growth plates (physis) is paramount in children, elastic nails are typically introduced in pairs. This configuration provides elastic intramedullary bracing and controlled micro-motion at the fracture site to promote callus formation—a gentler alternative to rigid adult nailing systems.

3.4. Reconstruction Femoral Nail

This line accepts either reconstruction femoral neck screws or standard femoral locking screws.

The reconstruction configuration is primarily indicated for femoral shaft fractures with concomitant ipsilateral femoral neck or proximal trochanteric injuries, stabilizing the diaphyseal fracture while securing the femoral neck and head.

4. End-to-End System Integration: Implants and Instrumentation

The VPEARL System includes:

  • Interlocking nails contoured for each anatomical site.
  • Comprehensive locking hardware: bone screws, femoral neck lag screws, anti-rotation helical blades, reconstruction screws, self-tapping screws, and specialty fasteners.
  • End caps and extension caps in multiple lengths.
  • Dedicated instrument sets for each nail type, complete with high-precision targeting jigs.

The VPEARL Interlocking Nail System is supplied in both sterile and non-sterile formats to accommodate the operating workflows of different medical centers.

The targeting jig guides accurate locking screw insertion—particularly for distal holes hidden from direct sight—shortening operative time and reducing intraoperative C-arm fluoroscopy exposure.
